COMPLETED CHRISTIAN SERVICE PACKETS
AND
SPONSOR FORMS (SIGNED BY THE PASTOR OF THE SPONSOR'S PARISH)
ARE DUE BY FRIDAY MARCH 2nd, 2012.
This will give time over the February break to complete hours and packets if necessary. Any student unable to download and print may pick up copies at rectory (please send an e-mail to janelle@beld.net to request in advance).
SPECIAL NOTE TO CATHOLIC HIGH SCHOOL STUDENTS: You will meet for 6 sessions from January - April. Your Christian Service from your school MAY be used for Confirmation but you MUST complete the Christian Service Packet and all other forms by the deadline.

~CONFIRMATION NAME FORM (you should choose a Saint name which you have researched
and write a brief paragraph explaining why you chose the name)
